Player Tryout Permission and Release Form

This form is to be utilized on behalf of all players from an association outside of the Northern Ontario Hockey Association,  who are attending tryout/training camps of hockey clubs with which they were not registered in the previous or current season.  If you belong to the NOHA and are planning on trying out for a team in the NOHA then you do not require a release.

Please allow a minimum of 2 business days for the release form to be processed.
Requests submitted after 4pm on Friday will not be viewed until Monday at 9am.
